The latest BlackBerry app allows for complete care coordination and visit management by home healthcare agencies. Care workers are able to view and update care plans for each patient they meet with, securely and in real time. The app takes advantage of the end-to-end security of BlackBerry 10, allowing for secure messaging and photo capture directly from within the app.
Messaging and photos updated in the app will alert other disciplines to execute and dispatch the level of care needed in real-time. This ensures critical patient updates do not fall through the cracks, leading to better patient outcomes.

The HCTP allows health care team members access to view their client’s clinical and demographic information all in one central location. Client information is stored securely, and only accessible by approved users via a registered login and password. Alerts are received on the device and also the SaaS based portal for a more efficient management of staff and added referrals.
